Coffee House Registration Forms

We are excited to begin another year of Coffee House fun, friendship, and faith.  Our new season starts Friday, September 19, 2025, and we look forward to sharing many wonderful evenings together.

An email will be sent out around the week of August 25th with your registration forms.  If you cannot find the email, you can download the information here at any time. 

To participate in Coffee House, we kindly ask that each caregiver/guardian or participant complete and submit a registration form before attending the program.  For new participants, we also like to schedule a brief phone call to discuss any specific needs and make sure we can best accommodate each participant.

Important Information:

• Participation is free.

• Registration is required prior to attendance (established and new members)

• Participants must be 18 years or older

• Your information will only be use for Coffee House purposes
